Congratulations on your pregnancy! Thank you for choosing Clinic Sofia OB/GYN, PA for your obstetrical care. We are happy to be sharing this exciting time with you. We hope that you find this information helpful to keep you on track during your pregnancy.

On this page you will find an OB appointment chart to help you determine when your appointment needs to be scheduled and who you will need to see, a prenatal health history form that should be filled out prior to your confirmation visit, prenatal billing information, and a prenatal packet that will be a useful source for frequently asked questions and information on your pregnancy. To pre-register with the hospital online, continue to the bottom of the page.

When scheduling your OB appointments at Clinic Sofia, please refer to the OB Appointment Chart to determine when you should be scheduling your next appointment and who you need to see. (MD for Doctor and NP for Nurse Practitioner)

Before you arrive to your first prenatal visit, please print off the Prenatal Health History Form and have it completed for the physician. If you are a new patient or have not been to the clinic within six months, please also complete the Patient Intake Form and Health History Form.

The Prenatal Information Packet is a useful guide for you to refer to when you have questions regarding your pregnancy. This packet is also available in your prenatal folder.
