Patient Forms

New patients to Clinic Sofia fill out a patient health history form and a patient intake form. These forms are scanned and stored securely in an electronic medical record storage system. Clinic Sofia has invested in the newest technological resources to insure your medical and personal information is kept secure and confidential.

We do our best to accommodate our patients needs; please notify us if you have any disabilities we should be aware of before you schedule your appointment.

If you wish to have your medical records transferred to Clinic Sofia from another facility, you must contact that facility and sign a Medical Record Release Form.

Established Annual Exam patients and new patients need to complete the following two forms. Please bring these with you to your appointment.

• Health History Form

• Patient Intake Form

If you are an established patient that has not been to Clinic Sofia within the last six months and are coming in for a consult, problem, or recheck appointment, please fill out the Interval Health History Form. If you are coming in as a new patient or for your annual, you do not need to fill out the interval health history form.

Patients transferring their care to Clinic Sofia need to contact their previous doctors office and request their records be mailed or faxed to Clinic Sofia. Patients who are transferring their care from Clinic Sofia to another physician should complete a Clinic Sofia Medical Record Release form and submit the signed form to the Clinic Sofia medical records department. Your request may be mailed to the clinic or faxed to 952-345-4448. HIPAA guidelines allow a clinic up to 30 days to process your request.
